BABY BIBS AND BOTTLES-WE ARE THE VILLAGE NFP SIGNUP FORM

Becoming a participant of Baby Bibs and Bottles-We are the village NFP, you will receive newly donated items as they become available.  Each enrolled child 8 years old and under, will receive a yearly birthday card with a age appropriate toy or gift card.  Parent(s) and/or Guardian will be able to participate in FREE raffles throughout the year. Also, enrolled households will receive a Free care package, twice per year, as they are available.  Processing completed forms may take 2-4 weeks and if approved you will receive a congratulations email.  We have the right to deny any signup form, at anytime, at our discretion with or without notice.
